RSR Financial Forms and Documents

These forms will help you organize your relevant tax information prior to filing your return.  Select the forms that are most related to your personal situation, print the forms to paper and fill them in with your relevant totals.  Once completed you can fax, email in (pdf), mail the forms to us or bring them in with you if you have scheduled an appointment.

The personal information form and source document forms are very important.  The personal information form is for you to list the legal name, social security number and date of birth for you, your spouse and all of your children and/or dependents.  The source document list form is a check list for the documents that will be sent to you and the IRS and must be matched when your return is prepared.

If we are going to help you secure financing of any kind, it is a good idea to let us run your credit bureau report first.  Most lending is driven by your FICO score and all lenders use specific scoring models to determine your credit worthiness.  If you would like us to run your bureau for you, we will do it at no cost to you.  Make your request by email, and we will send you a form to authorize us or our lenders to do this for you.  We will provide you with a consumer copy of your report when it is completed.

Once you receive your form (via email, fax or mail), just fill in your legal name, social security number, home address, date of birth, sign it at the bottom and fax it back to us.  We will fill in the remainder of the pertinent information if required.
